How to Shield and Detect PHI When using Generative AI

Introduction:

One can observe that the use of generative AI is expanding almost in every industry, including healthcare IT. However, the data collected in healthcare is often confidential and includes protected health information (PHI). According to the HIPAA rules, the PHI must not be sent to partners without a formal contract usually called a BAA, Business Associate Agreement. Since most of the LLM/Generative AI providers do not offer a BAA, PHI must be removed from all data sent to generative AI providers.  This article will discuss the AWS Detect PHI Service as an option to detect PHI from any text.  The AWS Detect PHI service employs sophisticated machine learning algorithms to identify PHI, help organizations meet healthcare compliance, and safeguard patient data.

The Growing Threat of PHI Breaches in Healthcare

Healthcare data should be kept confidential, and its integrity should always be preserved because a violation of the PHI results in serious consequences for patients and healthcare organizations. The Health Insurance Portability and Accountability Act has put measures in place to ensure that information in electronic health records does not leak out and that the country’s people continue to have confidence in the health care system.

However, the development of technology in health care and the use of electronic health records have brought new challenges to managing and securing the data. When healthcare data is transferred from one agency to another or from one platform to another, the risk of data leakage, breaches, and cyberattacks is heightened. However, the results also show that there is always a possibility of re-identifying the patients and their data, even when the data has been anonymized. It is crucial to work with a HIPAA-compliant company that has a Business Associate Agreement (BAA). This will help prevent and protect the PHI.

AWS Detect PHI: A Solution to Safeguard Patient Data

AWS Detect PHI is offered by Amazon Web Services (AWS), which can identify and mask PHI in text. PHI means any information that can lead to the identification of an individual and can be in the form of a medical history, diagnosis, or treatment, among others. The service applies NLP and machine learning algorithms to identify and redact PHI elements such as the patient’s name, date of birth, and other information in the free text.

To mitigate the aforementioned risks, healthcare institutions should develop integrated information protection policies that extend beyond the HIPAA statutes. The AWS Detect PHI Service is beneficial in this regard as it helps healthcare providers detect and conceal PHI on the data without necessarily being accessed or used by unauthorized persons.

Through the integration of the AWS Detect PHI Service, healthcare organizations can automate the operations of data management, increase the level of adherence to privacy regulations, and strengthen the protection of e-records as well.

Key Features of AWS Detect PHI Services

The AWS Detect PHI service offers several key features to facilitate PHI detection and extraction:

Text-based PHI Detection:

The text-based PHI Detection service is designed to thoroughly scan any textual data and reliably identify cases of PHI (Protected Health Information). It can deal with different types of inputs, including plain text, JSON, and HTML, due to its ability to cover all data types.

Multiple PHI Types:

It can identify all eighteen types of PHI, including the name of the individual, street address, social security number, medical record number, and many more. It is done by applying machine learning models tuned to healthcare data, which yield very high accuracy in detection.

Custom Entity Recognition:

Users can also build models to identify particular kinds of PHI for their field of interest and add to the service’s capabilities in this regard. This feature allows for the definition of the service based on the specific needs of the organization and the identification of additional categories of sensitive information.

Scalability and Performance:

AWS Detect PHI is one of the services offered by Amazon Web Services (AWS), where the main aim is to detect the presence of PHI in the text data. It utilizes AWS’s strong framework to parse and analyze a large amount of textual data. Due to the low latency and high throughput, the service can be applied both for real-time processing and, for example, batch processing, and therefore it is rather universal for healthcare and other industries.

Integration with AWS Services:

The service is easily integrated with other AWS services such as AWS Lambda, which is a serverless computing service; Amazon S3, which is an object storage service; and Amazon Compute Medical for the processing of unstructured medical texts. These features allow the consumption of data without much effort and its integration into pre-existing processes within an organization while enhancing operational efficiency.

AWS Detect PHI Use Case Scenario: Removing PHI Information from a Consultation Transcript

In this particular case, a doctor-patient consultation record needs to be forwarded to OpenAI for the generation of the SOAP note and the referral letter. Unfortunately, as of the writing of this article, does not offer BAA-based services. Thus any data sent to OpenAI needs not have PHI.  To ensure compliance and maintain the patient’s privacy, the AWS Detect PHI service can be used to accurately extract all PHI from the transcript before passing it to OpenAI.
